If there has been a divorce in your past, please download and complete the marriage questionnaire.
If there has been a bankruptcy in your past, please complete the bankruptcy questionnaire.
If you have had a felony conviction, please complete the felony questionnaire.
*If you are currently with another organization that is recognized by the PCCNA (Pentecostal Charismatic Churches of North America), we will need a letter of recommendation from the organization.
*If you have held credentials with the PCG in the past, we will need a letter of clearance from the district in which you were credentialed.
